**Open Position: Professional Nurse – Community-Based Health & Wellness Services**
This role supports the Western Cape Government Health (WCGH) Community-Based NPO program. The successful candidate will deliver essential clinical health services across Witzenberg's communities, including Wolseley, Ceres, Bella Vista, Nduli, Prince Alfred Hamlet, and Op-die-Berg.

### **Key Responsibilities**
#### **Patient Care Duties**
- Assess and monitor patient conditions, including chronic illnesses, wound care, and palliative support.
- Maintain accurate reporting and record-keeping systems.
- Develop and manage treatment plans in coordination with multidisciplinary teams.
- Implement infection control programs and advise on necessary precautions.
- Address patient concerns promptly to ensure quality care.
#### **Community-Based Services**
- Organize health outreaches and campaigns on HIV, TB, STIs, child health, and reproductive health.
- Administer contraceptives and immunizations as part of child health programs.
- Conduct cervical screenings and deliver a wide range of wellness services through mobile and fixed sites.
#### **Administrative and Team Management**
- Regularly train and supervise community health workers.
- Plan daily, weekly, and monthly team activities.
- Ensure accurate documentation and maintain high care standards.
- Monitor and manage staff performance and motivation.
- Maintain communication flow and accurate records in electronic or manual systems.

### **Minimum Requirements**
To be considered, applicants must meet the following criteria:
- **Education:** Matric (Grade 12) and a relevant Nursing degree or diploma.
- **Registration:** Registered with the South African Nursing Council (SANC).
- **Experience:** Minimum of 3 years as a Registered Nurse. Experience in community-based programs, palliative care, or wound care is advantageous.
- **Driver’s License:** Valid license with access to own transport (travel costs reimbursed).
- **Skills:** Strong communication, interpersonal skills, and the ability to work independently or as part of a team.

### **Employment Details**
- **Contract Period:** Fixed-term until 31 March 2025, renewable based on performance and funding.
